Overview

What liposuction is — and is not

Liposuction removes fat from beneath the skin through fine tubes called cannulas, inserted through incisions of a few millimetres. It is a contouring operation: it reshapes specific areas where fat sits stubbornly despite reasonable diet and exercise.

It is not a weight-loss treatment and it is not a treatment for obesity. The amount of fat that can be safely removed in one session is limited, and removing large volumes carries real risk. It does not treat cellulite, and it does not tighten loose skin — in some cases it makes loose skin more obvious.

Fat removed by liposuction can sometimes be processed and reused as a graft elsewhere, for example in fat transfer to the breast or facial rejuvenation.

How the operation works

A tumescent solution — dilute local anaesthetic with adrenaline — is first infiltrated into the area. This numbs the tissue, reduces bleeding and makes fat easier to remove. Fat is then suctioned through the cannula in careful, even passes to avoid leaving irregularity or contour dents.

Small volumes in a limited area may be done under sedation with local anaesthetic. Larger or multi-area cases are performed under general anaesthesia at an accredited private hospital, sometimes with an overnight stay.

Who it may suit

  • Adults at or near a stable, healthy weight
  • People with clearly localised fat deposits rather than generalised excess
  • People with reasonable skin elasticity, so the skin redrapes after fat is removed
  • Non-smokers in good general health

Who it may not suit

  • Anyone seeking weight loss, or whose weight is still changing significantly
  • People with substantial loose or hanging skin — abdominoplasty or a body lift may be more appropriate
  • Anyone with significant heart, lung, clotting or bleeding disorders
  • Smokers who will not stop

If loose skin is the main problem rather than fat, Dr Kueh will say so and discuss abdominoplasty, arm lift or thigh lift instead. It is common for the honest answer to be that liposuction alone would disappoint you.

What recovery involves

Bruising and swelling are considerable for the first two weeks and the treated area feels firm, tender and numb. A compression garment is worn continuously for around four to six weeks; this is not optional, as it controls swelling and helps the skin settle evenly.

Most people return to desk work within one to two weeks. Strenuous exercise resumes at around four to six weeks. Swelling masks the result for months — the contour usually looks its best somewhere between three and six months after surgery.

Risks and limitations

Risks include bleeding, infection, seroma (fluid collection), prolonged swelling, permanent numbness or altered sensation, and visible contour irregularity, rippling or asymmetry. Skin can occasionally become loose or uneven after fat is removed.

Less common but serious risks include blood clots in the legs or lungs, and problems related to fluid shifts or anaesthesia, particularly when large volumes are removed. These risks rise sharply with the volume treated, which is precisely why safe limits exist and are observed.

Fat cells removed do not return, but the remaining fat cells can still enlarge. If you gain significant weight afterwards, the shape will change again. Liposuction is not a substitute for maintaining your weight.

Liposuction FAQ

Is liposuction a way to lose weight?

No. It is a contouring procedure for localised fat, not a weight-loss treatment. The volume that can be safely removed is limited, and patients who are still actively losing or gaining weight are usually advised to stabilise first.

Patients closest to a stable, healthy weight tend to be the most suitable candidates.

Will the fat come back?

The fat cells removed do not grow back. However, the fat cells remaining in your body can still enlarge if you gain weight, and fat may then become more noticeable in untreated areas. Maintaining a stable weight is what preserves the result.

How much fat can be removed safely?

There is a limit, and it depends on your body weight, general health, and whether other procedures are being performed at the same time. Exceeding safe volumes significantly increases the risk of serious complications.

Dr Kueh will explain the appropriate limit for your case rather than working to a figure you have read elsewhere.

Will I have loose skin afterwards?

It depends on your skin elasticity. Skin with good elasticity generally redrapes well. Skin that has already lost elasticity — through age, pregnancy or major weight loss — may remain loose or look looser once the underlying fat is gone.

Where that is likely, a skin-tightening procedure such as abdominoplasty may be the better option, and Dr Kueh will tell you so at consultation.

Do I really need to wear the compression garment?

Yes. The garment controls swelling, reduces fluid collection and helps the skin settle smoothly onto the new contour. Patients who stop wearing it early tend to have more swelling and less even results.

How much does liposuction cost in Kuching?
